Job Description Email cover letter and resume NOW HIRING CER A NON PROFIT FOCUSING ON BEHAVIORAL HEALTH IS NOW HIRING FOR FULL TIME CASE MANAGERS Must be available am to pm Monday Friday This is a full time position We are hiring for Field Case Manager and Clinic Case Manager Field Case Manager requires a car Clinic Case Manager does not CHALLENGING POSITION As ac nonprofit agency working with the homeless mentally ill population we are Mission driven and our employees are motivated by the desire to help those who need it most We are under service and admin requirements from the State of Hawaii and other contractors This means our program is complex and continually changing It means that our employees have to be flexible and able to work within a challenging State and Federal system It sa challenging position REQUIREMENTS A minimum of a Bachelor s degree with related coursework is preferred At least months of full time equivalent experience employment or volunteer with any specialized population such as the mentally ill is preferred Candidates must have a history of stable and consistent employment Training is provided Must have a clean criminal background check as relates to the position Must have access to a vehicle and valid driver s license clean driving abstract and valid safety check Field CM only FULL COMMITMENT This is not a second job Candidates must be fully committed full time to the nonprofit agency Monday through Friday NATURE OF THE CASE MANAGER POSITION The Case Manager helps the mentally ill integrate into the community using a written Recovery Plan Obtaining psychiatrist visits medications housing substance abuse treatment etc It is a field position as opposed to working in an office Transporting clients is sometimes required This is a flexible position in terms of structuring your own workday Never punch a time clock again This position is for those with a social conscience who want to help people who have trouble helping themselves COMPENSATION FOR CASE MANAGER Training pay is per hour for hours per week for two to eight weeks until you build up your caseload After that pay is annually for a full caseload DEMANDING DUTIES A lot of paperwork and other responsibilities ALL progress notes must be submitted to the EMR within hours of service delivery ALL consumer Recovery Plans and paperwork must be updated minimally every six months ALL acuity Face to Face visits must be made or attempted during the month Treatment team meetings x per month clinical supervision x per month ongoing behavioral health training x per month of which are on line and meeting with Team Leader Program Manager x per week SUPPORT AND HIGH STANDARDS You have plenty of support from your supervisors and program managers You meet at least once a week with your program manager to review your Performance Matrix We are a small close knit agency and not a corporate giant We expect that all our employees perform in a top notch manner and give percent daily We hold all our employees to high standards BENEFITS Agency pays for HMSA medical dental prescription health benefits Cell phone or monthly stipend and laptop provided at no cost to the employee Currently the agency provides no mileage reimbursement Paid time off as long as client visits are made by you or by members of your Treatment Trio WORK WITH A LOCAL NONPROFIT WITH A CLOSE KNIT AND DEDICATED STAFF FOCUSED ON HELPING THE MENTALLY ILL INTEGRATE INTO THE COMMUNITY AND HELPING TO INCREASE THEIR QUALITY OF LIFE Email cover letter and resume NOW HIRING https cerhawaii org
