Description: Plans, manages, & directs daily operations of home health agency. Schedules, coordinates, & evaluates patient care services in assigned geographical area. Optimizes home health operations through effective leadership & Quality Improvement principles. Ensures quality patient care & resource management. Provides assistance w/ direct patient care as appropriate. Serves as resource to home health staff & patients. Supervises assigned staff.

Essential Functions:
- Supervises & evaluates field staff assigned to their team, including RNs, occupational therapists, social workers, speech language pathologists, home care aides, & management assistants.
- Schedules Home Health clinical staff. Promotes optimal productivity for Home Health staff. Assigns new referrals. Assures coverage of client care needs.
- Participates in recruitment & retention efforts.
- Schedules orientation of new clinical staff & coordinates orientation w/ Quality Coordinator. Responsible for each employee's complete orientation.
- Promotes growth & development of clinical personnel. Participates in competency assessment. Creates developmental/improvement plans. Identifies educational needs & recommends educational opportunities that would enhance patient care.
- Participates in & facilitates staff meetings, workgroups, committees, & other professional meetings. Communicates w/ other departments & community resources to coordinate care & services for Home Health patients & other Kaiser members. Models & promotes Kaiser's Standards & Behaviors & incorporates Standards & Behaviors concepts into meetings.
- Provides guidance to Home Health Staff & reports concerns to Home Health Manager in accordance w/ agency standards. Monitors patient care information.
- Coordinates & evaluates services provided to patients. Facilitates resolution of problems identified, & enforces adherence to patient care standards.
- Assists w/ developing, implementing, reviewing & evaluating clinical policies & procedures. Facilitates coordination of patient care. Facilitates communication w/ physicians & other caregivers. Utilizes resource personnel.
- Collaborates w/ Home Health management team to establish, implement, monitor, & evaluate administrative policies & procedures for home health service.
- Maintains open lines of communication to provide problem resolution w/ staff, patients, families, physicians, & other departments or agencies participating in care of KPHHA patients.
- Evaluates needs of patients & appropriateness for home health care & other services.
- Exhibits budgetary awareness in utilization of staff, contract services, supplies, & equipment. Researches & recommends purchase of new supplies & equipment. Assures medical supplies are sufficient & patient care equipment is functional.
- Assists in ordering & coordinating supplies & equipment for referred & active home health patients. Authorizes equipment based on medical necessity & member benefit. Advises appropriate person of equipment approximate costs to inform patients.
- Refers cases to outside agencies when scheduled Home Health Staff is maximized. Ensures continuity of care.
- Rotates evening, weekend, & holiday on-call to provide phone consultation & visits as necessary. Rotates weekend office coverage for patient referrals.
- Facilitates clinical expertise of students assigned to KPHHA, including scheduling, preceptor assignment, supervision & evaluation.
- Maintains availability, at office, or by pager/phone during scheduled work hours, & provides qualified alternate contact during any absence.

Basic Qualifications:
- Minimum three (3) years of physical therapy, occupational therapy, speech-language pathology or related experience.
- Bachelor's degree in physical therapy, occupational therapy or speech-language pathology OR four (4) years of experience in a directly related field.
License, Certification, Registration
- Valid Hawaii Physical Therapy, Occupational Therapy or Speech/Language Pathology license (must meet education requirement(s) for Hawaii State licensure).
- Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
- Valid driver's license.

Additional Requirements:
- Demonstrated knowledge of and skill in adaptability change management, conflict resolution, interpersonal relations, oral communication, problem solving, and written communication.

Preferred Qualifications:
- Minimum one (1) year of home health experience.
- Supervisory/lead or program management experience.
- Master's degree in healthcare related field in business and/or administration.
- Knowledge of community resources, home health reimbursement, and regulatory issues.
